## Beyond the Biomedical Model

For much of the 20th century, the **biomedical model** explained illness almost entirely through biological causes like pathogens, injuries, and genetics. In 1977, **George Engel** challenged this view with the **biopsychosocial model**, arguing that health emerges from three interacting domains.

| Domain | Examples |
|---|---|
| **Biological** | Genetics, pathogens, physiological state |
| **Psychological** | Beliefs, emotions, coping skills, behavior |
| **Social** | Socioeconomic status, culture, relationships |

The SAME diagnosis plays out differently based on context. Chronic pain, for instance, depends heavily on coping style, beliefs, and supportive relationships, not just tissue damage.

The **WHO** defines health as complete physical, mental, and social **well-being**, an explicitly positive standard requiring all three domains.

## The Power of Interaction

A **common pitfall** is treating the model as merely **biology plus a little psychology**. The framework insists the three domains genuinely INTERACT, meaning they actively shape one another rather than sitting side by side.

**Interactions in action:**
- **Psychological to biological:** Chronic stress raises cortisol levels and impacts immune function.
- **Social to psychological:** Social isolation significantly increases the risk of clinical depression.

Under this view, patient care shifts from merely treating physical pathology to addressing the full spectrum of human experience to achieve true health and well-being.

## Practice questions

7 of this lesson's 13 practice questions, with answers. The full set is in the app.

### 1. A GP asks a patient with chest pain: *"What's going on at home?"* Under the **biomedical** model that question is a waste of the appointment. Under the biopsychosocial model, what is it?

A. A courtesy that builds rapport before the real examination begins
B. An overreach, the psychological belongs to psychiatry, not general practice
C. A screening question for a separate mental-health referral
D. A diagnostic question, since those are causes and not context

**Answer:** D. A diagnostic question, since those are causes and not context

**Why:** The model earns its keep by changing what counts as data. If chronic stress raises cortisol and cortisol is doing some of the work, then 'what's going on at home?' is a question about the pathology, not around it. That is why the model rewrote the consultation, not just the textbook.

### 2. Select every statement that is TRUE about these models of health.

A. Social factors are irrelevant to health outcomes.
B. The biopsychosocial model treats the three domains as interacting.
C. The biomedical model explained illness mainly by biological causes.
D. The WHO's standard is positive - well-being, not merely the absence of illness - so biology alone cannot judge it.
E. The biopsychosocial model ranks biology as the only real cause.

**Answer:** B. The biopsychosocial model treats the three domains as interacting.; C. The biomedical model explained illness mainly by biological causes.; D. The WHO's standard is positive - well-being, not merely the absence of illness - so biology alone cannot judge it.

**Why:** The biomedical model's biological focus, the interaction of domains, and the WHO's positive definition are all true. The biopsychosocial model does not privilege biology as the sole cause, and social factors matter greatly - so those two are false.

### 3. Two patients tear the same knee ligament and have the same operation from the same surgeon. One is back at work in six weeks. The other is still disabled a year later, and no scan shows any difference between them. Why is this a problem for the biomedical model but not for the biopsychosocial one?

A. Identical pathology does not settle the outcome.
B. The biomedical model was only meant for infectious disease.
C. Psychological factors replace tissue damage as the cause.
D. The scans are not sensitive enough to find the difference.

**Answer:** A. Identical pathology does not settle the outcome.

**Why:** Same tissue, same repair, a year of difference, and the biomedical model has one place to look, so identical scans ought to mean an identical course. When they do not, it has run out of explanation. The other two domains are where the difference actually is: what the patient believes pain means, and whether they can afford to recover slowly.

### 4. Under the biopsychosocial model, two people with the same diagnosis can have very different illness experiences because of their psychological resources and social context.

**Answer:** True

**Why:** True. The same diagnosis plays out differently depending on coping style, beliefs, and social context - which is exactly why the model adds psychological and social domains to biology. Identical pathology does not mean identical experience.

### 5. A patient given an inert pill reports genuine pain relief, and brain imaging shows real endogenous opioid release. Which claim does the **placebo effect** support?

A. Belief substitutes for biology, so the biological account was unnecessary
B. A psychological state can produce a biological change, the domains genuinely interact
C. Nothing about the model, placebo is a measurement artefact to be subtracted out
D. The pain was never real, since a sugar pill removed it

**Answer:** B. A psychological state can produce a biological change, the domains genuinely interact

**Why:** Expectation reached down and released real opioids, belief is not sitting alongside the biology, it is acting on it. The trap is option 2: relieved pain was never imaginary, and a mechanism you can photograph is a poor candidate for 'not real'.

### 6. Order a causal chain that shows the biopsychosocial domains interacting.

**Answer:**

1. Social isolation deprives a person of support (social factor).
2. This raises their risk of depression (psychological factor).
3. Depression leads to poor self-care and elevated stress hormones (biological factor).
4. Physical health outcomes worsen as a result.

**Why:** Isolation (social) raises depression risk (psychological), which alters self-care and stress hormones (biological), worsening physical health. The chain shows the domains genuinely interacting - the model's core insight, and the answer to the 'side by side' pitfall.

### 7. Sort each factor influencing a person's health by its biopsychosocial domain.

**Answer:**

- Biological: A virus infecting the lungs, A genetic predisposition to diabetes
- Psychological: The patient's belief that recovery is possible
- Social: Low income limiting access to care

**Why:** Pathogens and genes are biological, beliefs and coping are psychological, and income and relationships are social. The biopsychosocial model insists all three domains jointly shape health - not biology alone.
